All athletic forms are submitted online through TouchBase for Prairie Trail.

Five Steps for Online Physical Registration

- Print the pre-participation physical form and take it to the physician's visit.
  1. Print English pre-participation physical form (PDF)
  2. Imprima el formulario de preparticipación física en español (PDF)
- Log in to Touch Base, choose your student and then choose "Registrations."
- Choose "Register" next to the activity or athletics registration tab, then choose "Manage."
- Complete and sign the concussion, student transportation and substance abuse forms.
- Upload the completed Pre-Participation Physical as one document and "Submit." You will be notified by email of acceptance. This is required only once, regardless of multi-sport tryouts.
Contact your school athletic office with questions.
Those who don't have a scanner can use their smart phone for the Pre-Participation Physical form.
- Download a scanner app.
- Snap a picture of each page of the document.
- Save all five pages as one document and upload it from your computer or phone.

All athletes using their own shoulder pads or helmet will need to have their equipment examined by the coaches and parents must sign a "Private Equipment" waiver.

24 - PRT Football FundraisingPlease share 5 emails of friends and family with the PRT coaching staff. Each person will be sent the below email from the team with a short description of our fundraising goals. The email will include information about our donations page, as well as, how to sign up for GiveBack2Schools. Also, please share the link to our donation page and GiveBack2Schools on your personal social media. PRT FOOTBALL DONATIONS. The more the word gets out the better. Below is what will be sent to each email. "Dear [insert name], I am fundraising to raise money to help support the PRT Football Team. I’m encouraged by the support we have already received, but we still need help reaching our fundraising goal. Would you be willing to make a contribution to my fundraiser? A gift of any amount would mean the world to me. To donate, simply click the link to my fundraiser below: PRT FOOTBALL DONATIONS It would also mean a lot if you could share my fundraiser link on social media. The more exposure I get, the faster I will reach my fundraising goal. Sincerely, [Your name]"

24 - PRT Football Equipment Check-outParents please fill out the information as you collect gear for your athlete. Athletes must check out with Coach Springer in the locker room before hitting submit. Game jerseys and pants will be handed out at a later date. ***DO NOT LEAVE WITHOUT HITTING SUBMIT*** **If you plan to use your own helmet or shoulder pads they need to be approved by Coach Jetton. You will need to sign an equipment waiver before participating** Uniforms are issued to you as a football athlete at PRT. All uniforms/equipment will be checked in at the conclusion of our season following the final game. The coaching staff expects you to maintain the PRT uniform throughout the season and care for them with respect to our program. Please only use bleach free detergent and never iron the uniform. All uniforms are the property of Prairie Trail Middle School. These uniforms are not for sale. At the conclusion of your season you will check in the uniforms issued to you or pay the replacement cost listed with each item.
